In the 1930s, to survive 10 years with childhood onset diabetes was a remarkable achievement. The cover image shows a medal issued by the Joslin Clinic which reflected the prevailing uncertainty about what the future might hold for these survivors; many did indeed die prematurely from diabetic nephropathy and coronary artery disease. Medals are now awarded for 70 year survival, but we still know relatively little about 'how to survive diabetes', the title of this month's Editorial (Diabetologia 52: 559-567).
